Pillar Health is a new community health center that provides integrated medical, dental, and behavioral health services in a health home model. This model places patients and families at the center of care delivered by a team and uses evidence-based approaches and referrals to resources to address their individualized needs.

This new facility at Washington Road is designed to support a high degree of collaboration and coordination between team members that are required to develop a personalized plan that is comprehensive and inclusive of both medical and non-medical needs. Patients may even see multiple members of the care team at the same visit.
