Patient Forms

Kenosha Community Health Center provides access to medical and dental care to over 15,000 patients through a team staffed with Doctors, Dentists, Advanced Practice Registered Nurses, and Hygienists.

Access for All

Kenosha Community Health Center and Pillar Health provide access to medical, dental and behavioral health care to over 17,000 patients annually through a care team of Doctors, Psychiatrists, Advanced Practice Nurse Practitioners, Dentists, Dental Hygienists, Registered Nurses, Clinical Social Workers and Licensed Professional Counselors.

We accept most forms of insurance and many of our patients have Medicaid. We see all community members regardless of their ability to pay by assisting them with Medicaid enrollment or discounting services based on income and family size.

Patient Support & Resources

Behavioral Health and Resource Specialists work to create opportunities for each patient to improve their health by providing education and resources to overcome social barriers outside the exam room. All clinics have bilingual staff (primarily Spanish) and access to other interpreters in over 30 languages for patients who need them. In addition, we also provide assistance to patients who are deaf or hard of hearing.

Be Prepared for a Doctor’s Office Visit

A visit to the doctor’s office is important. Make sure you are prepared to meet with your provider and care team.
